Location of Registration: Chicora Elementary Library (This includes Sugarcreek students) Age Requirement: Children must be five years old before July 1, 2020 to register for Kindergarten. Documents that are REQUIRED to be presented the day of registration:

• Child’s birth certificate • Updated Immunization record • Proof of residency such as driver’s license or utility bill that shows current address • Emergency telephone numbers

The Pennsylvania State Health Law requires each child entering school for the first time to have the following immunizations before school begins:

• Diphtheria and Tetanus (DTap, DTP, Td or DT); 4 closes, with at least 1 dose being on or after the fourth birthday • Polio (OPV or IPV): 4 doses, with at least 1 dose being on or after the fourth birthday • Measles/Mumps/Rubella (MMR): 2 doses with the first one on or after 12 months of age • Hepatitis B (HBV): 3 doses • Varicella (Chicken Pox Vaccine): proof of immunity; had disease or 2 doses of vaccination

The immunization requirements are a condition of attendance at any public, private, or parochial school in Pennsylvania. Parents will be expected to produce evidence of the immunizations or their child will be denied entrance to school until the requirements are met. ***Exemption for immunizations would include a written statement from physician that suggests medical exemption because an immunization would pose a health risk, or a written statement of religious or strong moral exemption from parent or guardian. Other required documents: The Department of Health mandates that a complete physical and dental examination be completed upon entrance into school. These dated examinations will be accepted up to one year prior to the start of school.

What to expect when you and your child go to Kindergarten Registration: You will complete many forms. We suggest parents pick up a registration packet ahead of time to complete the forms prior to registration day. Parents will move through stations that include: nurse, psychologist/counselor, transportation, & Title l.

Orientation activities for my district include: Just before school starts, parents and children are invited to a Kindergarten Orientation. This allows your child to get over those “first day jitters” before starting school. They will meet the teachers, go through lunch procedures, and even go for a bus ride! At this time, parents can meet the teachers, ask any last minute questions, and discuss transportation arrangements.
