Emlenton, PA — Primary Health Network (PHN), Pennsylvania’s largest Community Health Center, will open a newly renovated, permanent health center in Emlenton on June 2, 2025, as part of its ongoing efforts to expand access to care in rural communities.

Following several months of providing interim care through its Mobile Health Unit, PHN will transition to a fixed-site location at 516 Main St, Emlenton, PA 16373—the former site of Emlenton Area Family Practice. The upgraded facility will offer comprehensive family medicine services, including primary care, minor sick visits, medication management and education, and telehealth options where appropriate.

“Our investment in Emlenton represents more than just the opening of a new facility—it’s a long-term commitment to the health and well-being of this community,” said Dr. George Garrow, CEO of Primary Health Network. “We believe every community deserves access to high-quality, compassionate care. This renovation reflects that belief and an exciting step forward for Emlenton residents.”

PHN Emlenton Health Center will be led by Stacy Zacherl, CRNP, a Certified Registered Nurse Practitioner specializing in family medicine. Stacy began serving patients at PHN in January 2025. She holds a bachelor’s degree in nursing from Clarion University in Oil City, PA, a Master of Science in Nursing as a Nurse Educator from Chamberlain University (June 2019) and completed a Post-Graduate Certification Family Nurse Practitioner Program at Chamberlain in April 2024. Her clinical background and community connection make her an ideal provider for the Emlenton area.

Looking ahead, PHN has plans to expand the team of providers at this site, further increasing access to high-quality, community-based care for residents in the region. This growth reflects PHN’s ongoing commitment to meeting the evolving healthcare needs of the community and ensuring that patients have timely access to trusted, local providers.

To help ensure care is accessible to everyone, PHN will also offer free transportation services for patients who need assistance getting to and from their appointments.

As a Federally Qualified Health Center (FQHC), PHN accepts most insurance plans and offers a Sliding Fee Discount Program for eligible patients, ensuring healthcare remains accessible and affordable for all, regardless of financial or insurance status.

Scheduling Appointments:

Community members can schedule appointments by calling 724-704-4358. PHN encourages individuals and families to establish care early and experience the benefits of consistent, community-based healthcare.

This initiative reflects Primary Health Network’s broader mission to strengthen healthcare access in rural areas and improve health outcomes across Pennsylvania.
